Effective 9/1/2020, due to the time and medical expertise involved, Kitsap OBGYN, PLLC will charge a nominal fee for the completion of any forms (such as FMLA, disability, etc.).  This will be charged based on the length of the form and collected prior to releasing the form.    Please allow 7 business days to complete your form and we ask that you plan accordingly.   It will be your responsibility to complete your portion of the form and return it to the appropriate institution.   Thank you for your cooperation and understanding.

What is our Patient Portal?

On this web portal you may securely:

  • Confirm most appointments
  • Submit changes regarding personal demographic information
  • Communicate with your provider regarding medical questions, prescription refills, referrals and lab results*
  • Communicate with our office staff

*Lab results are typically available on the portal 96 hours after providers review and sign the report.

Billing and Insurance

There are thousands of insurance plans and they vary considerably.  There is no way for us to predict or guarantee if all of our services will be covered. Although we try to estimate what your insurance company may cover/pay, it is the insurance company that makes the final determination of your eligibility and benefits.  We suggest you contact them directly to get your benefit information and to make sure we are a part of the plan that your employer chose. 

***As of January 1, 2020, Coordinated Care and CHPW Insurance through Washington Apple Health will no longer be available in Kitsap County.   If you have either of these plans, and have not investigated new plan options, Kitsap OBGYN accepts Molina Apple Health and United Healthcare Apple Health.   We are NOT providers for Amerigroup Apple Health.  Please visit or call 1-855-WA FINDER (1-855-923-4633) for assistance in enrolling in a new plan!***
